The five categories of secondary evidence are delayed birth certificates, early public records, birth affidavit, letter of no record or foreign birth documents.

DS-11

After the applicant has complied all of the necessary identification documents that will be asked of them, it is time to move on to the actual application. The United States passport application is also known as Form DS-11. There are two ways to complete Form DS-11, either on the government’s website, or by hand in the application office. Basic identification information will be asked on the form. The basic identification information includes name, contact information and a physical description of the applicant.

Proper ID

Fill out DS-11, the application form, and return it to the proper agency office. If you are applying in person, the officer will ask you to provide identification with your application. The identification that will be requested is either a previously issued and undamaged passport, a valid id card or driver’s license, a naturalization certificate or a military id or government id.

Fees

Paying the appropriate fees is a necessary step to ensuring that a person’s application is processed. The 2011 price for an adult book and card is one hundred and sixty dollars. This price is made up of the application fee at one hundred and forty dollars, plus the execution fee, which costs twenty-five dollars.

Photos

A photo is required with every application. The requirements for the photo are simple. The pictures must be on quality color photo paper and the applicant’s face should be big enough to identify.
